Social Prescribing

Connect Well East Kent is a Kent County Council funded Social Prescribing and Community Navigator programme led by Social Enterprise Kent. This service is continues to be available across East Kent to all residents aged over 55 years, regardless of whether or not they are receiving any other services from Adult Social Care.

The service is also available to those under the age of 55 years, who meet the eligibility criteria and have complex needs.

The Community Navigation service is a community resource that aims to improve people’s quality of life, health and wellbeing by recognising that this can be affected by a range of social, economic and environmental factors

What is Care Navigation and Social Prescribing?

The Care Navigator role provides a proactive link between different services, being both a first point of contact for individuals, carers and health and social care professionals. It also guides and co-ordinates an individual’s journey through the care system.

Social prescribing is a way of linking people to sources of support within the community.

It provides health and social care professionals with non-medical / non-service focused options (outside of the care system) that can operate alongside existing treatments or care packages to improve health and wellbeing.
